Our providers want to make the most of your visit, so we require your completed new patient paperwork at least 72 hours before your initial appointment — we're unable to schedule until it's been received. Please return your forms to connect@atmaclinic.com.

Authorizes us to discuss your care with a family member or caregiver.
Medical Records Release
Authorizes us to obtain your historical medical records.
Nutrition Consent Form
Required if you're working with Ashley on nutrition as part of your care plan.
